INSTRUCTIONS FOR COMPLETING THE APPLICATION FORM

  1. It is very important that your application fields be completed carefully and accurately. The information you provide on these fields and any accompanying documents required will be used by the ACCL to determine your eligibility to sit for the examination. If accepted, your acknowledgement/confirmation letter and your examination results will be sent to the mailing address indicated on the form.
  2. Fill in all information requested.
  3. Candidates are not required to provide Social Security numbers other than on a voluntary basis. The numbers are useful as a secondary check in matching registration information to scores to ensure that they are reported correctly.
  4. Please remember that you will be required to attach or fax copies of certificates of attendance for the CME/CE activities that you submit for points and the titles of the activities must be clearly identified. If the title of the session is not clearly related to lipids, we also encourage you to submit a copy of the program or educational outline to assist with credentialing review. The Credentialing Committee will make the final determination on point assignments.

Note: Your name and the degrees/credentials you provide on your application is what we will use to produce your certificate if you successfully pass the exam. Please list only those degrees/credentials you wish to be present on your certificate.

Eligibility Criteria

Applicants must have achieved a minimum of 10 continuing education credit hours in clinical lipidology in the previous 2 calendar years. We encourage applicants to satisfy this requirement by completing the National Lipid Association (NLA) Lipid Management Training Course ( www.lipid.org/education/courses/lmtc).However, credit hours may alternatively be obtained by completing Volume I of the NLA-SAP ( www.lipid.org/sap), the NLA Masters in Lipidology Advanced Training and Board Review Course (www.lipid.org/education/courses/masters), or other courses of study in clinical lipidology which have been designed for successful completion of this exam.

* Note: If for any reason you do not meet the eligibility requirements, or if your cancel your examination (in writing), the examination fee will be refunded less a $120 processing fee.
